As soon as your appointment is confirmed, you will receive the appropriate questionnaire via e-mail. Make sure that you answer all the questions, especially the portion where you are asked about your expectation about the encounter.
PLEASE NOTE THAT YOU NEED TO FILL OUT THE QUESTIONNAIRE IN ITS ENTIRETY AND SEND IT TO ME AT LEAST ONE WEEK PRIOR TO OUR SESSION.
During the session, we will:
- Go over the questionnaire you completed
- Ensure that you understand the pros and cons of what has been offered to you. When you have a clear picture of what you want, it will be easier to work with your other health care providers.
- Develop clear action steps that will get you desired results.
- Create a personal action road map that you can easily follow afterwards.
